Often seen as an elite re-contouring procedure, the circumferential abdominoplasty will address the same issues as the extended abdominoplasty. However, with this procedure, excess skin on the lower back can be removed and the buttocks elevated at the same time.
Very often this procedure is used for patients who have lost significant amounts of weight, for instance after they've had bariatric surgery after a diagnosis of morbid obesity, with the resulting weight loss being over 70 pounds or more. The other reason that this procedure is requested is where there has been pregnancy or multiple pregnancies that have led to laxity of the skin and skin hanging or sagging in the abdomen, from the buttocks and over the flanks the hips and the lower back area. In the most severe cases of this, nothing other than this type of procedure is likely to achieve the desired result.
As with an extended abdominoplasty, the procedure is performed in a very similar way although for this procedure the patient will lie on one side and the incisions will be carried out to end the middle point of the back, Just between the cleft of the buttocks before being moved to the other side. The patient may also be operated on while they're on their stomach and then later in the operation turned over so that they're lying on their back.
The circumferential abdominoplasty is an effective procedure and quite often the results achieved by surgery would make you make you doubt that you were looking at that torso of the same person.
The patient’s skin will be marked to guide the surgeon on the amount of skin that is to be removed and the initial incision will be made. According to how much skin is planned to be removed the belly button may also be marked as it will be temporarily detached and repositioned after the operation, while the abdominal skin is being lifted, but will not be completely removed.
The skin of the abdomen will be lifted off the wall of the abdomen to reveal the musculature of the abdomen. The area to be dissected will extend to the flanks at this time.
The musculature of the abdominal wall will be tightened using running sutures and the muscle will be brought in to meet at the midline.
Now any excess skin will be elevated and pulled down. The area to be respected will previously been marked and this skin will now be removed along with the original how to the belly button which will now be below the line to be re-sected.
Next the wound in the flank will be exposed and the skin will be moved below and above and the excess trimmed following which the wound will be closed. The patient will be turned on their side and the procedure repeated on the other side.
When the procedure is completed the wounds will be dressed and the patient will be turned over on to the other side so that any excess flank skin can be removed.
To finish the procedure will be done again on the left side and the final closure and suturing will be done.
